Terms and Conditions

In consideration for being accepted by Coach Twitty Basketball, LLC (in person or online training/“clinic”/downloadable video program) & for participation in any basketball clinic sessions, I, being 21 years of age or older, for myself and on behalf of my child-participant, do hereby release, discharge, and agree to hold harmless Coach Twitty Basketball, LLC, Coach Joseph L. Twitty, all other clinic staff and volunteers, and the owners/operators of all facilities used for basketball clinic sessions from any and all liability, claims or demands for personal injury, sickness or death, as well as property damage and expenses, of any nature whatsoever which may be incurred by the undersigned and the child-participant that occur while said child is participating in the above-described activity. The undersigned further hereby agrees to hold harmless and indemnify said clinic, its directors, employees, volunteers and agents for any liability sustained by said clinic as a result of the negligent, willful or intentional acts of said participant, including expenses incurred attendant thereto. I hereby certify that my child is fully capable of participating in the sport of basketball and that my child is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities. Further, I agree that I will be present during all clinic activities and will supervise my child. I will monitor my child during clinic sessions to make sure that my child is, at no times, at any health risks in continuing clinic activities. I am the parent or legal guardian of this participant, and hereby grant my permission for him/her to participate fully in said clinic, and hereby give my permission to take said participant to a doctor or hospital in case of an emergency, and hereby authorize medical treatment, and assume responsibility of all medical bills, if any. I certify that all parents and/or legal guardians of the child have reviewed this release fully and have signed all forms recieved by Coach Twitty, as required prior to ( Privacy Policy for: purchasing & downloading ) Coach Twitty Basketball training videos from this site.
